9/26/2023 EnchantaVaughn Approved QMHPA arrived at the home to conduct individual session with the Ms. Cosby. QMHPA inquired about her morning. Ms. Cosby discussed with QMHPA her morning was going well. QMHPA informed Ms. Cosby this session was going to continue to assist her with increasing her ADLs and IDALs. Ms. Cosby did not present any symptoms of S/I or H/I at the time of the session. QMHPA begin session by inquiring about Ms. Cosby's emotional well-being. QMHPA reminded Ms. Cosby of appropriate tone and body language as she was preparing to go to the laundry. QMHPA assisted Ms. Cosby out into the community to assist her with increasing her IADL’s. While awaiting clothes to finish QMHPA discussed self-esteem and worked with Ms. Cosby on self-esteem and confidence building. During this time, QMHPA modeled appropriate communication skills and provided Ms. Cosby with an opportunity to engage in the use of appropriate communication skills. QMHPA also promoted healthy expectations and hope throughout the session. QMHPA discussed self-image and ways to improve your attitude and build confidence herself as well as brainstorm 5 things about herself on how she can better herself as a parent or better her parenting skills. QMHPA instructed Ms. Cosby to discuss with QMHPA positive traits in regards to herself and asked her to identify things she could change about herself if she wanted to change something. QMHPA processed each trait and encouraged Ms. Cosby to replace those negative thoughts with positive thoughts. QMHPA also reviewed how tone and inappropriate body language coincided with self-esteem and confidence as well. Ms. Cosby was open to suggestions and working on her anger for today's session. While exploring with Ms. Cosby her current emotional state, Ms. Cosby shared that she was in a positive mood. During the session, Ms. Cosby was able to actively participate in activity provided as she waited for her clothes to be completed. Ms. Cosby remained positive and patient as she stated that normally she would just take her clothes home and fold them but to remain somewhat organized she was going to stay and fold and separate her clothes. Ms. Cosby identified positive traits in regards to herself however she stressed the fact of her needing to gain trust for her daughter again and how to keep the angry that she has inside about her daughter and what she has done to her. Ms. Cosby also agreed to work on her tone and body language as she stated that negative body language could affect your self-esteem as well as your confidence level. Ms. Cosby stated that she wanted to work on being more lady like and having manners as she sometimes forgets and may utilize inappropriate language in the presence of others. Ms. Cosby informed QMHPA that she would try and be mindful of her language and tone from now
